Sponsor Evaluation
The Ohio Arts Council and its Arts Learning program are committed to ensuring the quality of all its programs
and services. This instrument is designed to gather information on the effectiveness
of the handbook and participant demographics. The results will be used in the
refinement of the handbook and other related activities.
This evaluation is designed to be printed using the Print Page link at the bottom
of this page. A separate window will open, choose Print from the File menu on
that window.
You also may download the PDF linked below,
fill it out on your computer and emaill, mail or fax it back to the OAC (you will not
be able to save the PDF with your information so please be prepared to complete
and print the evaluation when you open the document).
At this time we are not able to accept
completed evaluations online. Send the completed evaluation to
the Arts Learning Office, Ohio Arts Council, 30 E. Broad Street, 33rd Floor,
Columbus OH 43215 or FAX 614/466-4494. You may also email it to Arts
Learning Program Coordinator, Chiquita Mullins Lee at
